The Cleveland Clinic/Avon Lake Towne Center
450 Avon Belden Rd.
Appointments are required*

* Appointments will be taken starting July 1 for physicals starting at 5:30 pm. Please call the CRS office (440/933-5639) after July 1 to schedule your physical!
Please do not call The Cleveland Clinic to make an appointment. |
| EXAMINATION FORMS are available at your school athletic office, at the CRS office or online at www.avonlakecityschools.org/highschool/athletics/forms Click on Pre-participation Physical Evaluation. Please make certain that the health history section is completed and signed by a parent or legal guardian before the physical can be performed. |
| NOTE: Completed exam forms will be returned to the students for parent(s) review. It is the student’s responsibility to turn them in to the athletic office. |
| Proceeds from these physicals will benefit Community Resource Services (a 501(c) 3 non-profit). Cash and Checks (made payable to CRS) will be accepted. |

CRS will be accepting donations of new/packaged underwear & socks for our families with children ages 5 to 17.
|

a yearly physical with a regular healthcare provider***
